<p><strong>PUNE</strong>: Hernia is a common yet often overlooked condition that can significantly affect quality of life if not treated in time. It occurs when an organ or tissue pushes through a weakened area in the surrounding muscle. Factors such as heavy lifting, chronic coughing, obesity, and previous surgeries can increase the risk.</p>



<p>According to the World Health Organization, untreated surgical conditions, including hernia, contribute notably to the global disease burden. Data from the National Center for Biotechnology Information indicates that inguinal hernia affects nearly 27% of men and around 3% of women during their lifetime. The Ministry of Health and Family Welfare also highlights the importance of early diagnosis and timely surgical care to prevent complications.</p>



<p>Hernia often begins as a small, painless bulge in the groin, abdomen, or near a previous surgical site, leading many individuals to ignore it. However, early signs may include a dragging sensation, mild discomfort, pressure after prolonged standing, and abdominal heaviness or bloating. Recognizing these symptoms early and seeking medical advice is crucial.</p>



<p>Diagnosis is typically straightforward through physical examination, with imaging used in select cases. Early detection allows for simpler and more effective treatment.</p>



<p>“<em>In my practice, I have treated many patients who initially noticed only a small swelling. It is important to understand how this condition can progress if left untreated. With timely intervention, patients are often reassured by how straightforward the procedure is and how quickly symptoms such as swelling and discomfort improve after repair,</em>” said <strong>Dr Parmeshwar Bhambrule, Senior Consultant, Healing Hands </strong>Hospital.</p>



<p>Advancements in hernia therapy have made treatment safer and more efficient. Minimally invasive techniques involve smaller incisions, resulting in less pain, a quicker recovery, and a shorter hospital stay. The use of surgical mesh to further strengthen the affected area further reduces the risk of recurrence, enabling patients to resume daily activities comfortably.</p>



<p>Delaying treatment can lead to complications such as incarceration, where tissue becomes trapped, or strangulation, a serious condition requiring emergency care.</p>



<p>“<em>I always advise my patients that hernia treatment is not just about correcting a visible bulge but also about preventing complications and improving overall quality of life. <p><em>By Dr. Sana Ahmed Sayyad, Orthopaedic Surgeon – Shoulder &amp; Knee Specialist</em></p>



<p>As soon as winter arrives, Dr. Sana Ahmed Sayyad begins to notice a familiar pattern in his orthopaedic clinic. The number of patients complaining of knee and shoulder pain steadily increases, and the conversations start to sound strikingly similar. Many patients smile knowingly and say, “Doctor, the cold has found my knees again,” or “My shoulder pain always comes back in winter.” As an orthopaedic surgeon specializing in shoulder and knee injuries, Dr. Sana Ahmed Sayyad treats a significant number of patients with knee osteoarthritis and rotator cuff tears every winter season.</p>



<p>According to Dr. Sana Ahmed Sayyad, winter does not actually create joint problems—but it certainly amplifies existing ones. Joint pain that may be mild or manageable during warmer months often becomes louder and more persistent in cold weather. One of the key reasons for this is the drop in barometric pressure that accompanies colder temperatures. When atmospheric pressure decreases, the tissues inside and around joints can expand slightly. In healthy joints, this change is barely noticeable. However, in joints already affected by cartilage wear, arthritis, or tendon injuries, this subtle expansion can irritate sensitive nerve endings and worsen pain.<br>In patients with knee osteoarthritis, Dr. Sana Ahmed Sayyad frequently sees increased stiffness, swelling, and discomfort during winter. Many patients complain that their knees feel heavy, tight, or painful first thing in the morning. Simple activities such as getting out of bed, standing after prolonged sitting, or climbing stairs become challenging. Similarly, patients with rotator cuff tears or shoulder impingement often report worsening shoulder pain, reduced range of motion, and disturbed sleep, particularly night pain that intensifies in cold conditions.</p>



<p>Dr. Sana Ahmed Sayyad observes that elderly patients with knee arthritis are especially vulnerable during the winter months. Morning stiffness tends to last longer, walking distances are reduced, and fear of slipping often limits mobility even further. Prolonged sitting indoors, whether watching television or resting, allows joints to stiffen, making movement painful when activity is resumed. At the same time, younger and middle-aged patients with shoulder injuries frequently report sharp pain when reaching overhead, difficulty lifting everyday objects, and discomfort while dressing or sleeping on the affected side.</p>



<p>Another major contributor to winter joint pain, Dr. Sana Ahmed Sayyad explains, is reduced blood circulation. Cold temperatures cause blood vessels to constrict, which limits blood flow to muscles, tendons, and joint tissues. Reduced circulation means less oxygen and fewer nutrients reach already compromised joints, slowing healing and increasing stiffness. This is particularly problematic in the shoulder, where tendons such as the rotator cuff rely heavily on good blood supply to maintain strength and flexibility.</p>



<p>Cold weather also affects the muscles and tendons that support joints. Dr. Sana Ahmed Sayyad often explains to his patients that muscles become tighter and less elastic in colder temperatures. Tight muscles place extra strain on joints, increasing pain and the risk of injury. This is why flare-ups of shoulder pain and sudden knee discomfort are so common in winter, especially among patients who skip warm-up exercises or significantly reduce their physical activity.</p>

<p><strong>PUNE:</strong> Diabetes mellitus, commonly known as diabetes, is characterized by sustained high blood sugar levels and occurs either when the pancreas does not produce enough insulin or when the body cannot effectively use the insulin it produces. Major types of diabetes are: Type 1, Type 2, and gestational diabetes.</p>



<p><strong>A. Type 2 Diabetes</strong></p>



<p>Type 2 diabetes (T2D), which is the common type of diabetes in adults, occurs when the body cannot effectively use the insulin it produces (insulin resistance). Obesity, inactivity, unhealthy lifestyle, stress, and family history are the major risk factors for T2D. ‘No Symptom’ is the commonest symptom of T2D, as symptoms like excessive hunger, thirst, and urination, and weight loss may not be evident in the early stages. Therefore, routine checkup of blood sugar levels aids early diagnosis and timely treatment. 5 cornerstones of T2D management are – Diet, Exercise, Regular Monitoring, Medicines, and Patient Education. Uncontrolled diabetes can lead to damage to vital organs like the heart, kidneys, eyes, brain, nerves, and feet.</p>



<p>Diabetes Unit, KEM Hospital, Pune, provides comprehensive care featuring specialized services such as the Diabetes Renal OPD, Physiotherapy Clinic, Foot Clinic, Gestational Diabetes (GDM) Clinic, and Nutrition Clinic. <p><strong>B. Type 1 diabetes (T1D)</strong> occurs when the body’s immune system attacks the insulin-producing beta cells in the islets of the pancreas. As per IDF Atlas, India ranks 2nd in the list of top ten countries for the number of new cases of T1D (children and adolescents &lt;15 years), (11,300 per year).</p>



<p>T1D occurs most commonly in children and adolescents. People with T1D need multiple daily insulin injections to maintain a glucose level in the proper range and would not be able to survive without insulin treatment. With daily insulin treatment, regular blood glucose monitoring, a healthy diet, and lifestyle, these patients can live a healthy life and delay or prevent complications of diabetes. Diabetes Unit KEMHRC, Pune, has more than 1700 registered T1D patients.</p>



<p>The cost of treatment of T1D is a major issue, and for poor families, it is a very big drain on the family income. To address the need for treatment support as well as research, the Type 1 Diabetes in India (T1DI) project was initiated in the Diabetes Unit KEMHRC, Pune, with support from the Hinduja Foundation. Other NGOs like Mukul Madhav Foundation, Nityaasha Foundation and many individual donors support the treatment of underprivileged T1D patients.</p>



<p><strong>Patient engagement</strong></p>



<p>The Diabetes Unit at KEM Hospital, Pune, has established ClubOne KEM, a unique patient-support initiative dedicated to enhancing the quality of life of individuals living with T1D. This pioneering program fosters active engagement between senior and newly diagnosed patients, along with their parents and caregivers, creating a supportive and interactive learning environment.</p>



<p>The initiative aims to bridge the gap between routine medical visits by offering continuous guidance, encouragement, and peer-to-peer support. Through its activities, ClubOne KEM promotes confidence, self-management skills, and emotional well-being among young patients.</p>

These digital platforms provide ongoing education, real-life experiences, and expert advice.</p>



<p>The <strong>Madhuraksha mobile application</strong> was developed specially for T1D patients and their families. It is available for free on Android and iOS. This innovative app serves as a “pocket diabbuddy” for the patients. With over 2,000 downloads and nearly 800 active users, it provides vital information on insulin administration, glucose monitoring, travel tips, sick day care, FAQs, and educational articles in Marathi, Hindi, and English. The App also features calculators for insulin-dose adjustment and carbohydrate-counting, facilitating better blood glucose control.</p>



<p><strong>Diabag</strong>: ClubOne has also designed a special compact bag/ pouch (Diabag) which T1D kids can carry to school or during travel. It provides safe storage for all T1D essentials (insulin in the insulated cold compartment), glucometer, hypoglycemia management supplies, emergency contacts, etc., in one place. Diabag ensures prompt assistance in case of emergency, such as hypoglycemia (low blood sugar), when children are in school / away from home.</p>



<p>ClubOne KEM also conducts many <strong>workshops, competitions, and outdoor camps</strong> for T1D patients for patient engagement and empowerment. </p>

By moving the conversation offline, Dulcoflex® aims to spread awareness more widely in both urban and rural areas.</p>



<p>Constipation impacts 276 million Indians daily (Nielsen U&amp;A 2024 Functional + Perpetual). However, one third of those affected never seek help, and almost half rely on home remedies that may not always resolve the issue. <p><strong>PUNE:</strong> The Association for Diabetes and Obesity Reversal (ADORE), a Pune-based organization, has guided individuals in losing weight and controlling diabetes without medication through the Dixit Lifestyle. According to data from ADORE&#8217;s Diabetes Reversal Counselling Center, 3,500 patients have registered to lose weight or manage diabetes, with an average weight loss of 6.2 kg over six months. Out of these, 3000 patients are from Pune city and its surrounding areas.</p>



<p>Furthermore, approximately 398 diabetic patients have been able to reduce their HbA1c levels to below 6.5 without medication for at least three months, with 365 of them being from Pune and its surrounding areas. This information was shared by Dr. Jagannath Dixit, a renowned diabetologist and proponent of the Dixit diet. Dr. Dixit also mentioned that lakhs of people from India and abroad have benefited from the Dixit diet.</p>



<p>ADORE’s center has helped 35–40 patients with very high blood sugar to control diabetes without medicines, and 4 patients have successfully quit tobacco while following the Dixit lifestyle, he added.ADORE&#8217;s Trustee Balasaheb Kadam, Dr. Jayant Navrange, and Dr. Veda Nalawade were also present at the press conference.</p>



<p>The ADORE center in Pune has now been moved to a bigger space above the Mahindra Unnati Showroom in Apex Colony near Nal Stop on Law College Road, for the convenience of patients. This space has been provided by the charitable organization Ram Padma Godbole Foundation. The center will operate from Monday to Saturday between 5 PM and 9 PM. Dr. Dixit said that all medical check-ups and counselling services at this center will be completely free of cost. Also, this year, Infosys Foundation has once again extended financial support to ADORE’s social service work.</p>



<p>Dr. Dixit explained that Type 2 diabetes is caused by a bad lifestyle and can be controlled without medicine by eating two meals a day and exercising 45 minutes daily. Many patients have successfully managed diabetes this way, and it doesn’t require special foods or costs; it&#8217;s simple and free.</p>



<p>“<em>With financial help from Infosys Foundation, ADORE launched the ‘Infosys Foundation Diabetes Care Program’ centers in Pune and Nagpur in November-December 2022. A center in Nashik, funded by CTR Company, runs daily. ADORE also has centers in Thane, Kalyan, Sangli, Chhatrapati Sambhajinagar, Kolhapur, Solapur, Vile Parle, Surat, Belgaum, and Madgaon, operating one to three days a week. Recently, new centers opened in Badlapur and Bhayandar</em>”, said <strong>Dr. Dixit</strong>.</p>



<p>Based on patient data from the diabetes reversal center, numerous research papers have been published. One such paper, published in the Indian Journal of Community Medicine in February 2023, was honored as the ‘Best Research Paper’ of the year.</p>



<p>Considering the severity of the diabetes epidemic, there are plans to establish more such centers in cities across Maharashtra and India. For this, support from NGOs and corporate sponsors is essential. “<em>We are making special efforts to sustain these centers through community participation,</em>” <strong>Dr. Dixit</strong> concluded.</p>
