PUNE: In a major step forward in laser eye surgery technology, Worlds fastest and most advanced bladeless laser system (VisuMax 800 Femtosecond Laser System by ZEISS) has been installed at PBMA’s H.V. Desai Eye Hospital. This is the first such kind of installation in the state of Maharashtra. The information was given at a press conference by senior eye surgeons Dr Mukesh Paryani, Dr Varsha Puranik, and Dr Sanket Kulkarni.

Parvez Billimoria, Executive Director, and Dr. Rahul Deshpande, Chief Medical Director, H.V. Desai Eye Hospital, were also present on the occasion.

Dr. Mukesh Paryani opined that getting rid of glasses is more than a cosmetic requirement now. It has become a necessity in many professional and sports settings, where clear vision without the hindrance of eyewear can significantly enhance performance, safety, and confidence. One of the ways to get rid of glasses and contact lenses is to undergo laser procedures called refractive surgery. One of the major procedures used for refractive surgeries is SMILE (Small Incision Lenticule Extraction), which is used to change the shape of the cornea so that the light rays are focused on the cornea and the refractive error (glasses power) is removed. This latest technology uses ultrafast femtosecond ( tiny fraction of a second) laser pulses ( short bursts of light generated by lasers) to create a precise lenticule ( a disc-shaped piece of corneal tissue ), which is then removed. This is faster and safer and is an important milestone for refractive surgeries in Maharashtra.

Dr. Varsha Puranik said that the highly automated and robotic machine delivers higher precision, shorter procedure time, and less discomfort, resulting in faster recovery and better safety. This next-generation femtosecond laser system creates data-driven insights to help surgeons better manage treatment. While the machine is highly automated, the surgeon plays a crucial role in planning the treatment, guiding the laser application, performing the lenticule removal, and ensuring precision and patient safety throughout the procedure.

Dr. Sanket Kulkarni said that the patients undergo a comprehensive eye examination for eligibility. This includes Corneal tomography, Refraction, and visual acuity, and a complete, comprehensive examination of the eye. While proper precautions are necessary as prescribed by the ophthalmologist, most patients can resume normal activities within 3 days, with significant visual improvement noted within a day or two. Full visual recovery may take around 4 weeks.

Parvez Bilimoria, Executive Director, H.V. Desai Eye Hospital, said, “We have been at the forefront of delivering world-class eye care services. The new installation is a part of continuous advancements to bring cutting-edge solutions at affordable costs. It reflects our commitment to leverage the latest advancements in medical technologies with minimally invasive techniques and faster recovery times“.

Dr. Rahul Deshpande, Chief Medical Director, H.V. Desai Eye Hospital, said that “H.V. Desai is a comprehensive eye care institute, providing high-quality tertiary eye care in all sub-specialities, including Cataract surgery, Cornea, Glaucoma, Retina, ROP, Paediatric ophthalmology, Oculoplasty, Refractive error correction (LASIK). The hospital is recognised as a leader in comprehensive eye care, and has been providing all specialised eye care-related services in more than 10 districts of Maharashtra around Pune through a network of more than 40 Vision Centres.”

VOZ and SAFE were developed by Pune-based eye surgeon Dr. Jeevan Ladi transforming the field of laser vision

PUNE: The cutting-edge minimally invasive techniques for vision correction invented by Pune-based eye surgeon Dr. Jeevan Ladi are transforming the field of laser vision.

Explaining a recent case, Dr. Jeevan Ladi, Founder of Dada Eye Laser Institute said that a 30-year-old male from Sudan came to his clinic in the first week of January 2025. He was visibly dejected as all of the experts that he had met outside India told him not to go for laser vision correction as his cornea was thin. He had high myopia with a power of -7 in the right eye and -6 in the left eye, and the thickness of his Cornea, on which the Laser was done, was less.

Dr. Ladi added that the challenge in thin cornea is that there isn’t enough tissue to adequately correct refractive errors. However, the technique of Variable Optic Zone (VOZ) works by utilizing the concept of minimal tissue consumption depending on the size of the pupil. With the use of modern imaging technology, which uses a special type of laser light for accuracy, the size of the pupil is measured under different lighting conditions. This data is then utilized to decide the amount of tissue in the cornea that needs to be treated by Laser. It uses special software to do the necessary calculations to treat tissue, depending on the size of the pupil, to avoid photic phenomena such as glare and halos. This innovation has revolutionized the way we treat vision correction as it is tailored to each individual’s needs. The technique is suitable for people going for vision correction in the age group 18-60 years to get rid of glasses.

The procedure was performed on the Sudanese male on 4th January 2025. He regained his natural vision quickly and was very happy with the result. He went back to Sudan on 8th January 2025.

Dr. Ladi added that the other technique he uses is SAFE (Shearing Applied Force Extraction). This is a technique used in modern LASIK lasers such as Small Incision Lenticule Extraction (SMILE) to help minimize the amount of inflammation to the cornea and to aid in rapid healing and visual recovery. This method also eliminates the need for blades and flaps, making the surgery painless and reducing the overall procedure time. Those undergoing laser treatment for their vision can resume their regular activities almost immediately after the procedure.

VOZ saves more than 30% of tissue, increasing safety, while SAFE helps in regaining vision quickly. Both the techniques developed between 2022 and 2023 have helped many patients with safe vision correction. The articles authored by Dr. Ladi on these techniques have been published in reputed national and international journals.

What’s interesting is that some eye experts have also undergone these procedures. Pune-based private practitioner and eye surgeon Dr. Manasi Godbole-Gharpure said, “I was wearing glasses from my childhood for short-sightedness, also known as Myopia. I always wanted to get rid of my glasses. I underwent the innovative Laser treatment of Dr. Jeevan Ladi myself & could resume my routine work immediately. Not only the Indian population, who have relatively less thickness of the cornea but also people from all over the world will benefit from VOZ & SAFE. I could start eye surgeries on my patients just 4 days after my laser surgery.”

Cataract Surgery Accuracy Enhanced by The Ladi Method

Thousands of eyes and counting, “The Ladi Method” is helping detect the power of lenses in a cost-effective way with excellent results.

The Ladi method, another technique invented by Dr. Jeevan Ladi is keeping cataract surgeries within the reach of common people by providing a cost-effective technique to detect the power of the lens. The method received copyright in 2023 and has benefitted thousands of patients undergoing cataract surgeries.

Dr. Jeevan Ladi said, “Cataracts are one of the major causes of blindness in India. After the surgery, an artificial lens is implanted inside the eye for spectacle independence. The Ladi method uses ultrasound to find the power of the lens. Compared to other methods it is inexpensive and within the reach of common people, delivering excellent results.”

Dr. Ladi added that the natural lens of the eye is removed during the cataract surgery and replaced with an artificial one which is called IOL (Intraocular Lens). Pre-cataract surgery measurements are a critical part of the process and contribute to the outcome of the surgery. This helps in spectacle independence post-surgery as the power of Lens is calculated more accurately. The measurements are run through a formula to determine the lens to be used.

The method is inexpensive and accurate and therefore brings benefits to all sections of society. Another advantage of the method is that it can be used with any technique of cataract surgeries like Laser, robotic, phaco, and manual surgeries.

Prof Dr Mrs Ranjana Pande, an Eye Surgeon & patient, and former professor of the Department of Ophthalmology, at B J Medical College & Sassoon General Hospital Pune, said, “Since I believed in the technology, I underwent cataract surgery with The Ladi Method by Dr Jeevan Ladi. Now I am spectacle-free for all my activities in daily work. This Ultrasound-based method is inexpensive and will help not only developing countries like India but all sections of society the world over.“

First Metal Stent-Less Angioplasty in Maharashtra outside Mumbai

PUNE: A 70-year-old diabetic patient who had suffered a heart attack underwent Pune’s first laser-assisted, stent-less angioplasty at Sahyadri Super Speciality Hospital, Deccan Gymkhana. The revolutionary procedure, performed under the expertise of Dr. Abhijeet Palshikar, Director – Cardiology Sahyadri Super Speciality Hospital, Deccan Gymkhana marks a significant milestone in interventional cardiology.

The patient, who had a history of diabetes, experienced a mild heart attack three days before the procedure and was initially treated medically. However, doctors identified two key challenges, a heavy calcium buildup in his artery, which made traditional stenting difficult, and his reluctance to have a stent placed. To address these concerns, the medical team chose a laser-assisted angioplasty combined with a drug-coated balloon, which successfully opened the artery without needing a permanent metal stent.

The angioplasty procedure utilized an Excimer laser, a state-of-the-art technology that effectively clears calcium and cholesterol deposits within arteries. This was followed by the use of a drug-coated balloon, an innovative alternative to traditional stents. Instead of placing a permanent implant, the drug-coated balloon delivers medication to the artery and is then removed, leaving no metal behind.

Dr. Abhijeet Palshikar, Director – Cardiology at Sahyadri Super Speciality Hospital, Deccan Gymkhana explained the details and benefits of the procedure “The key benefits of the procedure include the absence of a permanent metal implant. Stents remain in the body permanently and may have risks of restenosis (artery re-narrowing). The laser-assisted approach eliminates this concern. Additionally, the patient experienced a faster recovery, being discharged within a day, walking independently, and resuming daily activities quickly. By the next day, he was mobile, walking independently to the restroom, and was discharged in the afternoon.”

“The success of this first-of-its-kind procedure in Pune paves the way for a new era in interventional cardiology, offering a safer and more effective alternative to traditional stenting methods which are minimally invasive alternatives.” Dr Sunil Rao, Chief Operating Officer and Medical Director at Sahyadri Group of Hospitals said.

The Excimer laser employed in the procedure is known for its precision in ablating calcified plaques, reducing risks while ensuring effective plaque removal. Additionally, drug-coated balloons provide targeted therapy, preventing restenosis without the need for permanent implants. With its many benefits, laser-assisted, stent-less angioplasty is expected to become a common procedure in the future, possibly replacing traditional angioplasty in many cases. It is not only useful for coronary artery disease but also for treating blockages in smaller blood vessels, where stents are less effective. The procedure is highly safe, with strict laser controls to reduce risks. It is also an excellent option for patients who have a blocked stent, as laser therapy can clear the obstruction without placing another stent.

“This breakthrough procedure offers a paradigm shift in angioplasty treatments. Patients who are hesitant about permanent stents or have complex calcified blockages now have an alternative that ensures better outcomes with reduced risks. The success of Pune’s first laser-assisted, stent-less angioplasty marks the beginning of a new chapter in cardiac care, and we anticipate its widespread adoption in the future.” Dr. Palshikar added.

Sahyadri Hospital is renowned for its exceptional cardiac care and commitment to integrating the latest medical advancements to deliver superior patient outcomes.
