<p class="wp-block-paragraph"><strong>PUNE:</strong> Ruby Hall Clinic announced a landmark advancement in regional oncology with the launch of the Indian subcontinent’s first Robotic High-Intensity Focused Ultrasound (HIFU) therapy. This cutting-edge, non-invasive treatment for prostate cancer will be housed at the newly inaugurated Kalyani Institute of Robotic HIFU Therapy at Ruby Hall Clinic in Pune.</p>



<p class="wp-block-paragraph">The facility was inaugurated by the leadership of Ruby Hall Clinic, including Dr. P.K. Grant, Dr. Simon Grant, and Dr. Himesh Gandhi, alongside the Kalyani family, represented by Dr. Baba Kalyani, Mrs. Sunita Kalyani, and Mr. Amit Kalyani of Bharat Forge. They were joined by international experts from Focal One, including Prof. Pascal (University of Toulouse), Dr. Damian, Vice President and CEO, and Mr. Ryan. The initiative has been made possible by the support of the Kalyani family, who have contributed to establishing the Kalyani Institute of Robotic HIFU Therapy at Ruby Hall Clinic, to improve access to advanced cancer care in India.</p>

<p class="wp-block-paragraph"><strong>PUNE:</strong> A Team of doctors led by veteran urologist Prof.Dr. Suresh Patankar and his robotic surgical team, including senior urologist Dr.Gururaj Padasalagi, Dr. Sachin Bhuiyan, Fellow Dr.  Saurabh Uplenchwar, have successfully performed a complex surgery of robot-assisted laparoscopic Pyelolithotomy with concomitant buccal mucosal graft ( BMG) Pyeloplasty in a patient with ectopic pelvic kidney. This new technique in surgery marks a major step in the use of robotic technology in complicated kidney conditions. It is believed to be the first such documented procedure in the world of robotic surgery. This has been confirmed by the prestigious Journal of Robotic Surgery, Germany.</p>



<p class="wp-block-paragraph">Giving details about the case, Prof.Suresh Patankar, chairman of Ace hospital, Erandwane, Pune, said that a 41-year-old man came to the hospital with abdominal pain and was found to have a kidney located in the pelvis, instead of its normal position. Along with multiple kidney stones, there was a blockage at the Junction, between the kidney and the joining ureter. Traditionally, such cases are extremely difficult to treat due to abnormal anatomy and the higher risk of complications.</p>



<p class="wp-block-paragraph">An ectopic kidney is a very rare birth condition that is often linked with ureter obstruction and rarely with kidney stones. Treating such cases can be difficult due to the unusual position and structure of the kidney, short ureter, complex blood vessels, and the vicinity of intestines makes the procedure challenging during surgery. While laparoscopic surgery is safer and offers faster recovery compared to open surgery, it can be technically demanding and time-consuming for surgeons.  Patients with long-standing kidney stones can cause inflammation, scarring, and make tissues friable; hence, any attempt at suturing for repair produces tearing of tissues, making surgery even more difficult and increasing the risk of bleeding, infection, and other complications. Earlier, medical guidelines recommended performing such operations in 2 stages. However, with robotic technology, complex surgeries have become safe, more efficient, and less stressful for patients and surgeons.</p>



<p class="wp-block-paragraph">Therefore, the Ace robotic team decided to go with a robotic surgery, added Prof. Patankar. Using a robotic surgical system, we were able to precisely remove the stones and repair the kidney in the same procedure. But this procedure was not simple and straightforward because of the multiple stones, and there was significant damage to the ureter. Hence, the challenge was how to overcome this problem, and there are no guidelines as such to repair this problem. So on the operating table, we decided to remove the stone laparoscopically and endoscopically, and simultaneously, using a buccal mucosa, that is, the skin inside the cheek, to repair the block.</p>



<p class="wp-block-paragraph">This patient recovered smoothly and was discharged on the 4 day after surgery. Follow-up showed a good result and no further blockage. A detailed review of existing medical literature confirmed that this is the first reported case where both procedures, stone removal and a graft repair, were done simultaneously on an ectopic pelvic kidney with robotic assistance. Our claim has been confirmed by a peer review of the prestigious &#8220;Journal of Robotic Surgery,&#8221; which stands amongst the top 25% of World databases.</p>



<p class="wp-block-paragraph">Dr.Patankar further added that hospitals have performed a large number of robotic procedures mostly for the kidney, prostate, including cancers, and reconstructions. But this procedure was a milestone and highlights how robotic surgery is revolutionizing urology, making even the most challenging procedures safer, precise, and more comfortable for both surgeons and patients. With greater flexibility of movement, enhanced 3D vision, improved ergonomics, and the wise thinking of the surgeons, it allows surgeons to perform complex operations with more precision and comfort. This gives a safe and better outcome for the patients.</p>

This technology helps ensure accurate bone cuts and optimal implant positioning, which are critical factors in reducing risks such as misalignment and instability. <p class="wp-block-paragraph"><strong>PUNE:</strong> Arcus Hospital based in Wanowrie today organized a special program to mark the milestone of 800 mako robotic surgeries for partial and total knee replacements and total hip replacements.  Dr. Rohit Luthra, Joint Replacement and Arthroscopy Surgeon at Arcus Hospital,  Dr. Juhi Luthra, and other dignitaries were present on the occasion.</p>



<p class="wp-block-paragraph">Dr. Rohit Luthra said that advancing robotic technologies is a boon to patients with partial and total knee and total hip replacements. Robotic systems are being used for greater precision in preparing bone for implant and accurately placing the replacement joint.</p>



<p class="wp-block-paragraph">Dr.Luthra added that lesser blood loss, no muscle cuts, and more accuracy have resulted in reducing the hospital stay by about half. </p>



<p class="wp-block-paragraph">A 3D scan helps develop a 3D model of the patient’s anatomy and is used by the surgeon to plan the surgery. The surgeon guides the robotic arm which aligns the implant to perfection and cuts only the portion that is needed and spares the bone tissues and surrounding muscles. The robot performs the procedure, with the surgeon maintaining overall control of programming and robotics.</p>



<p class="wp-block-paragraph">The Mako&nbsp;&nbsp;Surgical Robotic System is considered the most accurate and advanced robotic surgical system in the world for hip and knee replacements, including partial knee replacements which can be complex surgeries.</p>



<p class="wp-block-paragraph">Damage from arthritis is the most common reason for needing a hip replacement. Left unchecked, inflammation can lead to significant damage, causing a loss of function and disability. Arthritis can impact almost any part of the body, but most often affects the hip, knee, spine, or other weight-bearing joints.</p>



<p class="wp-block-paragraph"><strong>Direct Anterior Approach (DAA) brings much relief to the patients </strong></p>



<p class="wp-block-paragraph">Arcus Hospital was one of the first in India to do the robotic bilateral hip replacement surgery with the anterior approach in October 2023. Since then more than 650 surgeries have been done with anterior approach with or without robotic system. </p>



<p class="wp-block-paragraph"><strong>Dr. Rohit Luthara</strong> added that the Anterior approach or the ‘Front’ approach as it is called is a minimally invasive and muscle-sparing procedure. In this procedure, the incision is made from the front instead of the back or side. The damaged bones or cartilage are removed without detaching muscles or tendons and are just pushed aside instead of cut to allow the implant to be placed. This results in minimal damage to the muscles and since the muscles are not cut, the patient can sooner bear his or her weight and start walking. The robotic system adds pinpoint accuracy to the procedure including placing the implant into the right position. It helps us get the exact offsets as per the patient&#8217;s native anatomy of the hip and there is minimal chance of implant malposition and the risk of dislocation is nearly zero.</p>



<p class="wp-block-paragraph">One of the most crucial parts of robotic procedures is planning which ensures accuracy and safety for patients.  The sizing of the implant is done with a CT scan before the operation. So we do a CT scan, we get a 3D virtual model of the hip and then we see what implant will best fit the patient and according to the anatomy the surgery is planned for the Robotic system to execute it to perfection. Also, one very big advantage is that the limb length can be checked easily with this approach because the patient is lying supine ( flat on the back with face up) and not lying on the side.                  </p>



<p class="wp-block-paragraph">This approach is proving to be a game changer, he said.</p>

<p class="wp-block-paragraph"><strong>PUNE:</strong> Doctors at Ruby Hall Clinic, Pune successfully treated a 65-year-old patient suffering from a complex and large midline hernia through robotic-assisted surgery. The elderly patient presented with extreme pain and discomfort due to the enlarged hernia bulges and also had multiple co-morbidities, including asthma, hypertension, and diabetes.</p>



<p class="wp-block-paragraph">Given the patient’s age, multiple co-morbidities, high BMI, and a history of three vertical C-sections, the complexity of the surgery was significantly high. To ensure the best possible outcome, the patient underwent robotic-assisted surgery, using the da Vinci system which offered a more precise, minimally invasive approach to manage the challenges of the surgery.&nbsp;</p>



<p class="wp-block-paragraph"><strong>Dr. Suprashant Kulkarni, Sr. Consultant, General and Robotic Surgeon at Ruby Hall Clinic </strong>who led the surgery emphasized the importance of a multidisciplinary approach to improve surgical outcomes. “<em>Our primary goal was to optimize the patient’s overall health before the surgery. As the patient was diabetic, we made sure that her blood sugar levels were under control before the surgery to avoid any risk of infections. With the help of a pulmonologist, we were able to optimize her respiratory issues, effectively improving her condition for two months. As urinary and constipation issues are a common concern in elderly patients, addressing and managing these issues was necessary to ensure that the patient was in good condition for the procedure, thereby reducing the risk of complications</em>,” he said.</p>



<p class="wp-block-paragraph">The patient was advised to High protein, fiber-rich diet preoperatively, in addition to exercises like walking daily for 30-60 min, and to use inspiratory spirometry to optimize her lung and heart status.</p>



<p class="wp-block-paragraph">Highlighting the significance of optimizing the patient’s health before surgery, thereby improving the outcomes, Dr. Suprashant Kulkarni, further said, “Umbilical hernias, which affect both men and women, are more common in women due to the weakening of navel scar or weakening of vertical midline incisions (of Caesarean section) and surrounding abdominal muscles during pregnancy. In this patient’s case, a history of three vertical C-sections contributed to weakened abdominal muscles causing a large abdominal bulge (Diastasis recti) in addition to multiple incisional hernias (Called the Swiss cheese defects due to the typical shape caused by the defects).</p>



<p class="wp-block-paragraph">To prevent hernias in women, women need to focus on strengthening their abdominal &amp; other core muscles in not only in their growing age but also during antenatal and postnatal periods (as per recommendations from their Drs)”.</p>



<p class="wp-block-paragraph">A robotic platform ensures precise and accurate surgery and the least trauma to the patient’s physiology. The patient was walking on her own in the wards just 5.5 hours post-surgery, despite the long time required for the surgery, and was discharged within 48 hours post-surgery with minimal pain, tolerating a full oral diet</p>



<p class="wp-block-paragraph">Expressing her gratitude, the <strong>65-year-old patient</strong> said, “<em>I’m extremely grateful to Dr. Suprashant Kulkarni and his team for helping me recover this soon. They helped me understand the benefits of robotic-assisted surgery and explained how it can significantly reduce the chances of recurrence in the future</em>.”</p>



<p class="wp-block-paragraph">&#8220;Robotic technology, with its minimally invasive approach, is the best surgical treatment for addressing hernias. The three-dimensional vision, precision, and exceptional reach of instruments, offered by the da Vinci robotic system, enabled the correction of a large hernia through very small incisions. Any other surgical method would have required larger incisions, increasing the risk of infection, recurrence, and delayed wound healing,” Dr. Kulkarni added.&nbsp;</p>

<p class="wp-block-paragraph"><strong>MUMBAI:</strong> AIIMS New Delhi, a premium medical institute in India, and Intuitive,<a> </a>a global technology leader in minimally invasive care and the pioneer of robotic-assisted surgery, today signed a Memorandum of Understanding to establish an innovative new training centre for robotic-assisted surgery at AIIMS, New Delhi.</p>



<p class="wp-block-paragraph">The AIIMS da Vinci RAS training centre will equip surgeons and care teams with the skills and technology training necessary to perform robotic-assisted surgery across specialties including urology, gynecology, general surgery, and more. This industry-academia collaboration will advance the adoption of robotic-assisted surgery in India by providing state-of-the-art technology training for more surgeons.</p>



<p class="wp-block-paragraph">While signing the MoU,<strong> Gary S. Guthart, CEO, of </strong>Intuitive, said, &#8220;<em>For Intuitive, this MoU with AIIMS represents a significant milestone in our shared commitment to advancing surgical training and improving patient outcomes. We believe the AIIMS da Vinci RAS training centre will also serve as a beacon of excellence, fostering training, skill, and knowledge development for the next generation of surgeons.</em>”</p>

<p class="wp-block-paragraph">Intuitive’s commitment to training surgeons is not limited to initial technology training. It extends to providing ongoing support and training for surgeons and care teams throughout their careers, and as they advance in their use of RAS. As part of Intuitive&#8217;s continuing effort to establish strong robotics programs across India, the company has already established three training centres in different parts of the country. More than 850 surgeons in India have been trained in da Vinci technology to date and are helping drive a significant increase in its adoption.</p>
